Investigation Summary
At approximately 6:50 a.m. on February 8, 2002, Employee #1 was lubricating a carrier bearing on an auger conveyor using a hand-powered grease gun. He received contusions, lacerations and multiple fractures to his right ankle and foot. He was hospitalized and the right leg was amputated below the knee.
