On March 1, 2001, a supervisor had just restarted the machine and noticed a piece of debris in the bed of the machine. He attempted to remove the piece of debris before the bed of the machine and the top plate of the machine came together. The machine was not deenergized or locked out when his hand got caught between the bed and top plate. He suffered three severed fingers of his right hand. He was hospitalized for his injuries.
