Investigation Summary
At approximately 12:30 p.m. on August 8, 2000, Employee #1 was working under a Crown electric pallet jack that was elevated on the forks of a Toyota forklift truck. The pallet jack slipped off the forks, pinning him to the pavement. Employee #1 died of crushing injuries to his chest.
