WORKER FELL FROM LADDER FRACTURED ARM

On January 22, 1998, Employee #1, a maintenance worker at Sylmar High School, was attempting to descend a two-section extension ladder from the roof above the cafeteria. Employee #1 was carrying a bucket of "mastic" (roof patch). When Employee #1 stepped onto the ladder the top section slid down to the bottom section. The extension hooks were not properly engaged and the ladder was not tied off to prevent it from becoming displaced. Employee #1 fell to the ground breaking his left arm between the elbow and shoulder. Employee #1 was taken to Holy Cross Hospital for emergency treatment.
