Investigation Summary
At about 5:00 p.m. on January 23, 1998, Employee #1 was instructed by his supervisor to brown coat on the right side of the back of the building. Employee #1 climbed a scaffold approximately 25 feet in height to begin brown coating. After five minutes of brown coating, the wooden plank slipped off the metal scaffold. He fell to the ground and suffered multiple fractures to his back. Apparently, the metal scaffold did not have mid-railings and single planks. Employee #1 was a construction laborer for the general subcontractor A.A. Allied Construction, Incorporated.
