Employee's ankles fractured in fall from haystack

Employee #1, a regular, full-time employee of Eight Star Equipment Company, was placing a protective tarp over the top of a 20 ft high stack of baled hay. Employee #1 was climbing to the top of the haystack, and was approximately 16 ft up when he fell. He sustained two broken ankles. He was not wearing fall protection. The cause of the accident was failure of management to provide and enforce the use of fall protection. The general nature of the place of employment is farming.
