Investigation Summary
Employee #1, an experienced produce harvester with A & A Grove Service, was climbing a 28 ft ladder to harvest avocados from a 30 ft tree. He had climbed the ladder approximately 25 ft and was beginning to harvest the avocados when his foot apparently slipped off the ladder cleat. Employee #1 fell 25 ft through the tree, striking tree limbs in his fall to the ground. He suffered blunt head trauma and was killed. Apparently, the ladder cleats were wet from the previous night's rain, and Employee #1's tennis shoes slipped on the wet surface.
