Investigation Summary
At approximately 7:30 a.m. on January 28, 1999, Employee #1 was injured when his head was caught between the cage and mast of a forklift. Employee #1 was in the space between the driver's cage and the mast of a forklift to assist in a mechanical repair of the forklift. The forklift engine was running at the time. Facing forward Employee #1 sat on the tilt lever of the forklift. This caused the mast to tilt backward catching his head between the cage and the mast. Employee #1 was hospitalized for his injuries.
