EMPLOYEE'S FINGERS AMPUTATED IN POWER PRESS

Employee #1 was trying to unjam a part from a power press. Employee #1 stopped the machine but failed to lock it out. He removed the guard and placed his hand into the point of operation. The machine cycled crushing his hand and amputating his fingers. Employee #1 was hospitalized for his injury.
