Employee injured in fall from ladder

On May 23, 1999, Employee #1, a clerk, had ascended a Cotterman Metal Wheeled Platform Ladder with wheel locks to return merchandise to an upper shelf. The wheels were to be blocked. As Employee #1 descended the ladder he had merchandise in his hand, was not maintaining 3-point contact with the ladder, and was facing away from the ladder. Employee #1 fell, broke his wrist, and was hospitalized for three days.
