Employee suffers amputation of toe after it is crushed

At approximately 3:15 p.m. on July 29, 1999, Employee #1 was injured when a roll of plastic sheeting fell on his toe in the cover stuffing department. Due to a medical condition, the injury resulted in the amputation of the right big toe and several days of hospitalization. It was determined that the rolls of plastic sheeting weighed from 70 to 80 lbs. each. Prior to the accident, several rolls were stored on end and secured with a single cord. After the accident, the employer had plywood bins built to hold the rolls to prevent a similar accident from occurring in the future.
