Employee injured in fall through floor opening

At approximately 3:06 p.m. on August 15, 1996, Employee #1 was performing layout work for the construction of interior metal stud walls. He was chalk-lining a second-level concrete floor for placement of metal stud floor plates when he stepped on an unmarked 3/4 in. thick by 12 ft 10 in. long by 33 1/2 in. wide plywood sheet. The panel was covering a 12 ft 6 in. long by 30 in. wide air duct opening. Employee #1 was stepping on the sheet when it moved, and he fell 19 ft 4 in. onto the first-level concrete floor. He sustained multiple head injuries and was transported by paramedics to a nearby hospital, where he died on August 17, 1996. The plywood sheet had not been secured.
