Employee's foot crushed in forklift accident

Employee #1, age 19, of Disney Studios, was part of a crew assigned to demolish a building. They were working from the forks of a 11,125 lb Clark forklift, model C5004 55CP, serial #Y1015-34-7011CB, as directed by the forklift operator. The operator moved the forklift and raised and lowered the mast and forks while the crew was on them. Employee #1 was standing on the mast of the forklift, at a height of approximately 10 ft, when debris on the ground caused the forklift to rock when it moved. His right foot was crushed and his big toe was amputated.
