Investigation Summary
Employee #1 was cleaning a foot stand that was 10-ft-long by 27-in.-wide by 13-in. high using a power hose. Employee #1 picked up the grate with his left hand and held the hose with his right. Employee #1 attempted to place the grate back onto the frame when it unexpectedly slipped. Employee #1's left middle finger was caught between the metal frame and the metal grate. Employee #1 suffered an amputation of the left third finger tip. Employee #1 was treated for his injuries but was not hospitalized.
