Investigation Summary
Employee #1, a 19-year-old trainee, was hand feeding a piece of red oak into the wood shaper, holding it in place with a spring clamp on a jig fixture. The piece jammed and rotated clockwise, forcing Employee #1's left hand into the cutting head. His middle, ring, and little fingers were amputated; his middle finger was surgically reattached. Employee #1 had worked only one shift on the wood shaper machine prior to the accident.
