Employee's leg fractured in fall with ladder

Employee #1 was performing routine safety checks in the warehouse main service yard when he noticed a water hose connection leaking badly on the deck mat on the south wall. He was approximately 3/4 of the way up a 30 ft extension ladder that was leaned up against the deck mat when the bottom slipped away from the wall, probably due to the wet surface. Employee #1 fell with the ladder and suffered a fractured lower left leg. The ladder was not tied off, as Employee #1 assumed.
