Investigation Summary
At 8:00 a.m. on August 28, 2017, Employee #1 was line boring the lower bucket sleeve on a CAT 992 loader using a CAT 9U5730 Portable Boring Bar Unit. He was holding an internal micrometer in the hole after the first pass, and needed to adjust the horizontal position of the boring bar. As he tried to activate the hydraulics with his other hand, he pulled up the wrong button on the line boring machine control pendant and started the drill that rotated the boring bar instead. The drill was plugged into the hydraulic unit separately, but was not unplugged or locked out. Employee #1 sustained open fractures to his right hand and lacerations to his right forearm. He was hospitalized for his injuries.
