Investigation Summary

Investigation Nr: 201155231
Event: 10/18/2001
Employee injured when fingertip amputated by pan rollers

At 6:15 a.m. on October 18, 2001, Employee #1, a printing press operator, employed by Block Graphics, suffered a partial amputation to the index finger of his left hand. Employee #1 was injured while clearing water and sludge from the pan roller water tray with the pan rollers turning. While sliding a round metal drain plug along the bottom of the tray, in close proximity to the turning rollers, the drain plug momentarily contacted the rotating rubber roller. This created an in-going nip point between the bottom of the water tray and the rotating pan roller. The metal drain plug bound up and kicked back, severing Employee #1's fingertip. Coworkers came to his aid and drove him to the Menifee Valley Medical Center for emergency medical care. Employee #1 was transferred to the Loma Linda University Medical Center for additional medical care where he was treated and released. Employee #1 returned to work on October 25, 2001.
