Investigation Summary
At approximately 5:30 a.m. on August 13, 2001, Employee #1, a laborer employed by Northwest Pipe Company, sustained varying soft tissue injuries to his left elbow and forearm after slipping and falling 43.25 inches from a loading platform, while he was removing protective paper wrapping from 90- to 100-pound drums and cylinders of solid coal tar. While on the ground, a drum part struck Employee #1's forearm and elbow. Employee #1 was transported by helicopter to the Arrowhead Regional Medical Center, where he was treated and hospitalized for 5 days.
