Investigation Summary
At approximately 11:15 a.m. on April 28, 2001, Employee #1 was driving a fork-lift, a Yale Model GL 030, Serial Number N494323. While backing up, he made a sharp turn and the fork-lift tipped over to the left. His left hand and foot were pinned between the frame of the fork-lift and the ground causing multiple fractures on his hand and foot. Employee #1 was hospitalized.
