Investigation Summary
At approximately 3 p.m. on December 3, 2000, Employee #1, a carpenter who works for CA Hoffman Construction, broke his upper femur after falling twelve feet to the ground while setting up a tower scaffold. His injury was reported to the Division on December 4, 2000 at 10:20 a.m. by a customer service representative of Silvertone Insurance. On the day of the incident, four employees, including the foreman, were setting up a tower scaffold to build a shaft wall for an exhaust. The tower scaffold measured 10 feet by 5 feet and 6 feet 8 inches high for a single stage. They set up two stages of the tower scaffold on the floor. They built the shaft frame on one side, and they had to move the scaffold to the other side of the shaft. Two employees were standing in a cooler room. The cooler room measured 9 feet 9 inches high. Employee #1 was standing on a conduit on a metal stud wall while holding another conduit on the wall about twelve feet above ground level. They were in the process of moving the scaffold to the other side so they can set up the scaffold on the top of the cooler room next to the scaffold. While doing that, the conduit in Employee #1's hands gave way. Employee #1 fell to the concrete floor and fractured her upper femur, which required surgery on his hip at a local hospital.
