Employee Injures Finger and Hand on Conveyor Belt

At approximately 11:30 p.m. on October 20, 2000, an employee was working in Fast Flow Door Number 429, on a powered conveyor, Stewart Glapat Corporation, Serial Number 875260-2, Model Number-2580T24SPINCEL, speed 90 FPM. Just before the accident, the employee was arranging his paper work and a fan mounted on the door was running. The fan blew the papers away underneath the conveyer belt. As he went to grab his papers, his right index finger got caught between the belt and roller, causing severe laceration on his palm and fracture on his index finger. The employee was taken to the Arrowhead Regional Medical Center by paramedics where he was hospitalized for 36 hours. The nip points on the belt and roller on the conveyor were not guarded at the time of accident.
