Investigation Summary
At approximately 6:00 p.m. on October 23, 2000, Employee #1 was operating a trimming press, KPT-16, Serial Number 198-A7/00, to trim round parts. He was using two hand controls and had trimmed 70 to 80 parts. He was cleaning the table with his left hand and as he tried to reach a part with his right hand and the machine cycled. His hand was crushed between the ram and the table. He stated that he did not pull the safety bars. He received an amputation of his left thumb, index, middle and ring finger and was hospitalized.
