EMPLOYEE SUFFERS FRACTURES WHEN PINNED BY FORKLIFT

At approximately 4:19 p.m. on July 24, 1998, Employee #1 was operating a forklift when he decided to get off and help a coworker. Employee #1 did not set the parking brake. The forklift was parked on an inclined and it started to roll. Employee #1 attempted to jump on the forklift but it pinned him against the wall. Employee #1 received a fractured pelvic bone and was hospitalized.
