Employee's fingertip amputated in ribbon mixer

Employee #1, age 19, was in the dry compound room blending a dry powder mixture in the ribbon blender. At the end of the batch preparation, the dry powder mixture accumulated at the discharge chute of the blender, blocking the free-flowing movement through the chute. The procedure utilized to ensure the flow of product material was to manually deactivate the blender and to then use tools to free the product buildup. Employee #1 failed to deactivate the blender before using his left middle finger to circle the rim of the discharge chute. His finger became entangled in the mixing blade, amputating the tip.
