Investigation Summary
On July 8, 1996, Employee #1 was beginning his inspection of a work area when an air knife, a device that uses compressed air to blow excess zinc from steel during a galvanizing operation, fell from a storage rack onto his leg. The workers remove the air knife every 10 to 14 days to clean, inspect, and repair the unit. On the day of the accident, an overhead crane had lifted the unit from the zinc tank and placed on the storage rack. As the chains were being hoisted up and away from the air knife, one apparently became caught on a name plate or other projection on the unit. The device began to rock back and forth and then fell off the rack to the floor. It struck Employee #1's leg, pinning it against the floor. He sustained crushing injuries, for which he was hospitalized.
