Employee's foot crushed by lift truck

At approximately 11:35 a.m. on November 30, 1996, Employee #1 was working as a lift truck operator at a distribution center for Toys R Us in Fontana, CA. He went into an aisle between storage racks to move some pallets. Another specialized lift truck was also in the aisle, lifting pallets from the rack and placing them on the floor. Employee #1 got off his lift truck to scan the product that was on the pallets that he was going to move. As he was scanning the codes on the boxes, the other lift truck operator began to move his specialized truck into position to take more pallets off of the rack. Employee #1 could not get out of the way because of pallets that were on the lower level of the rack, and he was struck by the other lift truck. He sustained crushing injuries to his foot, and some of his toes were later surgically amputated.
