Employee's arm crushed in machine rollers

At approximately 10:00 p.m. on December 8, 1995, Employee #1 was cleaning and stripping polyethylene film from a set of rollers associated with an Amoco fan folder machine. His left hand became caught between the rollers, and his left arm was pulled into the nip point between two rollers. He sustained injuries, for which he was hospitalized for 16 days. The employer was cited for a serious violation of T8CCR 3314(a), because the fan fold machine had not been shut down and de-energized during servicing and cleaning.
