Employee's leg amputated in grain auger

At approximately 7:15 p.m. on May 18, 1995, Employee #1, of O.H. Kruse Grain & Milling, was cleaning dirt and grain debris from the bottom of a large grain storage structure. His right leg contacted a section of unguarded auger that amputated his lower leg. The employee was cited for a serious violation of T8CCR 3314(a) because the unguarded auger system was not effectively shut down or de-energized to prevent movement during cleaning or servicing.
