Employee's fingertip amputated by falling shaft

On October 7, 1994, Employee #1, of Van Can Company, attempted to catch a large machinery shaft as it rolled off his workbench. When he reached out with his hand to catch the shaft, his finger became caught between the edge of the workbench and the bottom of the shaft as it started to drop. Employee #1's fingertip was amputated and he was hospitalized.
