Investigation Summary

At approximately 7:45 p.m. on August 9, 2004, Employee #1, a sales associate at a retail clothing store, was stepping over a velvet stanchion rope that extended across the entrance doorway of her place of employment when the heel of her shoe got caught on the stanchion causing her to fall. The stanchion was draped across the doorway entrance at approximately 18 in. above the floor. Employee #1 was going into the area to empty a container of ice-water into a sink in an adjacent room. Employee #1 was taken to the hospital and treated for a fractured ankle.
