Employee Crushed When Flatbed Overturn

At approximately 4:00 pm, on November 8, 2004, Employee #1, a concrete subcontractor, was welding underneath an 18-foot long, 8-foot wide roll up detached flatbed. The employee had elevated the front end of the flatbed 5-feet using a forklift, and had added a circular steel pipe on each side of the front of the flatbed for additional support. Each pipe was approximately 5-feet long and 1-inch diameter. The rear end of the flatbed was at ground level and stationed with a 2x4 block of wood placed behind each of the rear wheels to act as wheel stops. Employee #1 was kneeling on his left knee with his right knee bent while welding, when the load began to shift. The employee tried moving out from underneath the flatbed, but he was not fast enough and the flatbed fell on top of him. The detached flatbed weighed approximately 2,700 lbs. Employee #1 was hospitalized with fractures to his spine, shoulders, neck, a crushed right femur, also lacerations to his head. Employee #1 underwent surgery to implant a permanent metal rod into the femur and eight staples in his head. The Orange County Fire Authority reported the accident. The direct supervisor was not at the site at time of the accident.
