Employee Fractures Leg in Fall from Ladder

On September 27, 2005, Employee #1, a coworker, and an independent contractor/truck driver had finished loading a resin underground storage tank onto a flatbed truck. Employee #1 climbed a straight, 14-ft Bauer ladder that was leaning against the tank to a height of approximately 5.5 feet to make an adjustment. Not checking that all equipment and personnel were cleared of the truck, the driver started the vehicle and began to slowly move forward. Employee #1 yelled to the driver to stop, but the driver did not hear. Employee #1 believed that he would hit his head if he went down with the ladder, so instead chose to jump to the ground, landing on his right leg, fracturing his tibia. Employee #1 was hospitalized.
