Employee's Foot Is Crushed When Run over by Forklift

On March 6, 2006, Employee #1, who works for Intel Corporation, was attending a training session with other employees. Employee #1 and his coworkers were in the exhibition area during the set-up of the exhibits. Employee #1 was standing with his back to an aisle, which was near the entrance of a display booth. A coworker was driving a forklift, a Toyota Model Number 7FGU25, Serial Number 73413, weighing 9,430 pounds, to move around display materials. The coworker parked the forklift near the employees and proceeded to exit the forklift. The parking brake was not set. The coworker's jacket caught on the gearshift. The gear shift moved into the reverse mode, and the forklift backed over Employee #1's foot. The coworker stopped the forklift when the wheel was on top of Employee #1's foot, and then put the forklift into the drive mode to move the forklift off of Employee #1's foot. Employee #1 sustained multiple fractures and crushed bone injuries to his left foot and ankle. He was hospitalized for treatment.
