Investigation Summary

Investigation Nr: 202550448
Event: 07/07/2009
Employee's Arm Is Fractured Caught in Bailing Machine

On July 7, 2009, Employee #1 worked at a business recycling carpet padding and carpet. They receive or pick up used padding and carpet, bale and ship it for recycling. They have three baling machines. Employee #1 was on a second shift, baling carpet in one of the balers, a Kilkom baling press, Model Number KV60HD, Serial Number 19010. The ram had begun to move downward, and he noticed that some of the carpet was crooked. He opened the safety gate and while the ram was still operating, reached into the baler to straighten out the carpet. His right hand became trapped between the carpet and the wall of the baler, and as the ram moved downward, it dragged his arm into the baler as well. A coworker hit the emergency stop button, then reversed the ram, freeing Employee #1's arm. Employee #1 was hospitalized with a fractured arm.
