Investigation Summary
On April 6, 2007, Employee #1 was helping to transport roofing materials to a job site, which was a residential home that was almost complete. Instead of using the inside staircase, Employee #1 picked up an extension ladder, a Louisill, Model Number LGT 20, and placed it against the garage. The ladder extended 12-in. above the garage roof and was not secured. As Employee #1 ascended the ladder, the ladder moved, and he fell approximately 44-in. The fall resulted in a strain and contusions to his back. He was hospitalized for three days.
