Investigation Summary
At approximately 3:10 p.m. on September 6, 2003, Employee #1 was working for DJ's Feed & Pet Supply, Inc. The employee and his supervisor were stacking hay bales in the back storage area of the feed store. The supervisor lifted three hay bales up on a forklift to the top of seven vertically stacked hay bales. Each bale of hay is approximately 16 to 18 in. high which made the total height from the ground to the top bale 124 in., or 10.33 ft, Employee #1 was removing the four hay bales from the elevated forks of a forklift while standing at the top of the seven high stacked bales of hay when he lost his balance and fell backwards to the ground, an asphalt road. The employee's head and right shoulder hit the asphalt. He was taken by ambulance to the Children's Hospital in San Diego, California. Employee #1 was hospitalized for treatment for an epidural hematoma, a fractured skull, and a separated A/C joint in his right shoulder. The blow to the head caused the blood vessels surrounding the brain to tear. He was required to have head surgery to relieve the swelling in the brain caused by the torn vessels. The surgery entailed staples being placed outside his head when the hematoma was evacuated.
