Investigation Summary
At approximately 5:15 p.m. on August 2, 2004, Employee #1, a service technician of the Shelburne Services, Incorporated, located in El Centro in California was attempting to service air conditioner on the roof of a residential home located in Calexico, in California. He was supervised by a supervisor. Employee #1 extended a ten foot extension ladder and began to climb up to the roof. The ladder slipped off the edge of the roof while he was standing on the third rung from the end of the extension ladder. Employee #1 fell 7 ft to 8 ft to the cement driveway. He fractured his right ankle in two places. Employee #1 was transported to Pioneer Memorial Hospital, where he underwent two different surgeries to his right ankle. Employee #1 was hospitalized.
