Investigation Summary
At approximately 9:30 a.m. on October 18, 2006, Employee #1, a carpenter, was using a Skill saw, Model 5860, Type 1, Serial Number HD-135955, 4300 rpm. He was making an overlap cut on a 4-ft by 8-ft plywood sheet at the panelized roof of building A. The saw jumped and contacted his left hand. He sustained an amputation of his left little finger up to the second knuckle, and a deep laceration to his ring and middle fingers. Employee #1 was hospitalized.
