Fork of Forklift Strikes and Amputates Worker's Toe

At approximately 6:45 a.m. on January 25, 2006, an employee was adjusting a detachable fork of a forklift truck. The employee was adjusting the fork gap on a Hyster Number 3, Model Number S115Xl forklift, which was equipped with detachable and adjustable forks. The employee was pulling the left fork, weighing 360 lbs., towards the left side, when it slipped off and fell on his left foot, causing the amputation of his big toe. He was taken to the Glendora Surgical Center, in Glendora, California, where surgery was performed. The employee was hospitalized for approximately 12 hours.
