Investigation Summary
At approximately 6:45 p.m. on January 9, 2006, Employee #1 was feeding strands of nylon into the in-running side of rolls of a CUMBERLAND, Model Number 6, pelletizer-machine with his gloved left hand. While feeding the strands, Employee #1's gloved left hand was pulled into the in-running rolls, causing laceration of his left hand and amputation of his left ring, middle and index fingers. Employee #1 was hospitalized for three days.
