Employee's Foot Fractured Between Forklift and Post

At approximately 11:50 a.m. on February 1, 2007, Employee #1, a new warehouse associate, was operating a forklift in the Receiving Department of the warehouse in Building 2. The employer receives, warehouses, and distributes a large variety of house ware products that they manufacture and buy from vendors at other locations. Employee #1 had been practicing on a Raymond Model 740-DR32TT high-reach lift truck and backed into a corner guard post. Just prior to the impact, she had inadvertently put her foot out and it was crushed between the forklift and the post. Her foot was fractured and she was admitted to Arrowhead Regional Medical Center for surgery and five days of in-patient treatment.
