Investigation Summary
At approximately 11:40 a.m. on July 11, 2002, Employee #1 and the employer were at a residence to trim a palm tree. Employee #1 was on the approximately 75-foot-tall palm tree cutting and the employer was on the ground supervising. Employee #1 wore a pair of spikes to climb the tree with a synthetic rope wrapped around his bare waist and the tree branch. While employee #1 was on the tree near the top, the employer observed Employee #1 untying the knots made on the rope that looped around the tree branch, which confined him from falling. The employer screamed to not to untie it but Employee #1 fell onto the concrete driveway and was killed.
