At approximately 6:31 a.m. on July 17, 2004, Employee #1 was stocking shelves from a Crown Work Assist Vehicle (WAV) elevated work platform and fell approximately 6-ft, 8-in. to the floor. Employee #1 sustained injuries to his ribs and back, and cuts to the back of his head and left leg. Employee #1 was hospitalized.
