Employee Slipped On A Bundle of Tubes - Fractures Arm

At approximately 3:20 p.m. on January 16, 2002, Employee #1, a packaging and fitting machine operator for Norse Dairy Systems. She was carrying a bundle of plastic tubes to another station, and the plastic tubes suddenly slipped out of the bundle and spilled to the ground. Employee #1 slipped on the tubes and fell forward, hitting her right arm above the elbow, fracturing her arm. She was hospitalized and had seven screws implanted into her arm during surgery.
