Employee's fingertip amputated when caught in conveyor belt

Employee #1 was adjusting a conveyor belt when it started to move. His hand became caught between the belt and a bar that lay across the end of the belt, amputating the tip of one finger. Employee #1 had not switched off the conveyor belt before starting to work on it.
