Employee's fingers amputated between lift and storage rack

Employee #1, a forklift operator, had just placed a pallet of materials on a storage rack and was maneuvering the truck to turn it around. While backing up, she had her head turned to the rear and her left hand on the frame of the forklift. She backed into the storage rack and her fingers became caught between the rack and the frame of the forklift. He third and fourth fingers were amputated at the first knuckle.
