Employee injured when struck by flying particle from press

Employee #1 was operating a 32-ton mechanical press forming small components for aircraft fasteners. Having completed one operation, she inserted another piece into the press. As the press stroked, a small metal particle from the previously formed piece came flying out and lodged in her heart. The piece had to be surgically removed.
