Investigation Summary
Employee #1 was operating a four-wheel rubber tired tractor. He was backing a sheepsfoot down a 2:1 slope to compact fill dirt. He lost control of the equipment and it rolled downhill and overturned. The sheepsfoot tamper broke off and the tractor rolled over several times to the bottom of the slope. Employee #1 was thrown out of the tractor and over a steep drop. He died at the scene. It cannot be determined if operator error or mechanical failure caused the accident.
