Investigation Summary

Investigation Nr: 125899377
Event: 02/12/2003
Employee fractured wrist while working on pneumatic press

At approximately 10:00 p.m. on February 12, 2003, Employee #1 was operating a pneumatic press machine. She was installing steel bearings to a pump assembly using the pneumatic press machine when one of the bearings slipped out of her hand onto the back of the table area. She retrieved the bearing, and her hand and the bearing came in close proximity or contacted the magnetic sensor switch, which activated the ram and suddenly pinned her wrist between the ram and bearing placement fixture. Employee #1 sustained serious crushing injuries to her right wrist. She was taken to Loma Linda Hospital, Loma Linda, CA., for treatment. On February 13, 2003, at 1:00 a.m., Employee #1 had surgery to repair her injured wrist. She was hospitalized for six days.
